CIMA is committed to caring for patients' needs throughout their hospitalization by using our hospitalists. Our hospitalists specialize in internal medicine or family practice and are members or affiliates of CIMA. Hospitalists are doctors who oversee patients' care while they are in the hospital. They are usually specialists in internal medicine or family practice but also specialize in hospital care. Because the hospital is one of their primary sites of practice they are more familiar with the hospital system and its function.
This Notice of Privacy Practices is being provided to you as a requirement of the Health Insurance Portability and Accountability Act (HIPAA). It describes your right to access and control your protected health information along with our legal duty to protect this information. Your "protected health information" means any of your written and oral health information, including demographic data that can be used to identify you.

Dr Maier advised me to get a CAT scan. I did and read the results on the sparrow portal. the results were not good. I called and left several messages asking a nurse or a dr to explain what these results meant. I was told by assistant to make another appointment. But when I called I had to wait 2 weeks to get another appointment. It would have been respectful just to call me and explain results. When I read my CAT scan results I was very upset. I became even more upset to be treated so poorly.

I would give 0 stars if I could. PA Jacqueline Lewis should not be allowed around patients nor should she be around people at all. Basically told me my medical conditions (that I've lived with for years) are fake. "Just drink water".yeah, heard that one before. ADA Federal Law was NOT followed. I am ADA Certified. I will die without my medicine. Hope this place goes out of business before they hurt someone.

ZERO stars from me & my daughter, but it wouldn't let me post unless i marked one. Just tried them as a new doctor office. PA Jacquelyn Lewis is horrible. Refused to treat unless we went to specialists because of medicine we have been on for years. We have already went to specialists. That's how we were prescribed the medication in the first place. We are both ADA certified. Those Federal laws were not adhered to. Don't waste your time going here.

I was able to get an appointment within about 3 weeks of my initial call so I was grateful. The office sent multiple reminder emails that included the location and time of appointment. I showed up to the space about 15 minutes before my appointment and spoke to a few friendly receptionists. As I began to fill out my paperwork, about 5 minutes before my appointment time, I was called back to the desk to hear that my appointment needed to be rescheduled because I was not 30 minutes early to the appointment.

She also communicates that the doctor is backed up, which makes sense, yet I am the one that is sacrificed due to receptionists over extending doctors schedules? I asked the receptionist if that policy was communicated and she said that she apologizes if it was not BUT here I am suffering from an unknown issue only to be rejected an appointment due to the lack of communication from the folks at this office.

This arbitrary policy was just being communicated to me over 15 minutes past my appointment time. Why wasn't this policy clearly stated in the multiple emails that were sent? Why wasn't this policy communicated when I first showed up on 15 minutes early rather than the 30? Why did the receptionist mention the doctor being "behind" if the policy is to be 30 minutes early?

So after asking these questions, without satisfactory responses, I left without rescheduling. The receptionist stated she was "sorry" and I rebuked her claim. I'm not surprised by this act but do want others to know my experience. I wish you all well in being served at this office.

I have never had such an awful experience with a doctor's office or any other business for that matter. A close friend recommended Dr. Amit Ghose and I sought him out specifically for my psychiatric and primary care needs. I spent three months checking in to see if he was able to take me on as a new patient and was eventually told he was willing to accept me as his patient and I scheduled an appointment.

When I showed up, I was told that I would not be seeing Dr. Ghose, I would be seeing a Nurse Practitioner named Adam Farnum. This threw me completely off-guard because every time I had called I had specifically requested Dr. Ghose and when they finally accepted me as a patient they told me Dr. Ghose would be the one taking over my health care. At this point, the receptionist acted very annoyed that I wanted to see the doctor that I had made an appointment with. Having had already taken off work to come into the office, I reluctantly saw Adam Farnum that day.

After contemplating the experience, I called the following day and explained that I had been misled and treated rudely and no longer wished to receive health care from their medical practice. Had that been the end of it, I would not be writing this review today. I have now received a bill for $150 even though I have Blue Cross Blue Shield health insurance which is accepted by their practice.

I called my insurance company explaining the situation and they thought this was very strange that I should be paying $150 out of pocket so they called Capital Internal Medicine to sort out the billing issue. After talking to Capital Internal Medicine, my insurance provider told me that because of the way Adam Farnum billed it, I was still responsible for $150.

She even said that they could rebill it in a way that my insurance would cover it but REFUSED TO DO SO. I have no idea why a new patient visit where I basically made a simple introduction and no medical procedures or tests were performed would result in me having to pay $150 when I have very good health insurance. Adam even said during the visit "the first appointment is basically a meet and greet".

My gut feeling is they are intentionally making me pay out of pocket because they are upset that I did not stay with their practice. I can't imagine any other reason they would refuse to rebill my SINGLE APPOINTMENT in a way that my insurance would cover it.

I left a message when trying to se up an appointment, (which i mean how hard is it to answer the phone? its kind of your job as reception?), and forgot to include some info. I received a snarky sarcastic message on my phone from Amanda. Unprofessional and rude. Now i have been trying to have my appointment set up for over 4 days. unacceptable

My care provider here had been good, until very recently. I have come to realize this office is not an ideal place for LGBTQ+ patients. The thought processes of the staff generally seem to be stuck in the 1980s, making assumptions about correct pronouns not being important, and equating HIV with LGBTQ+ care. I would hope these medical professionals would understand that's not a "gays only" issue. To suggest otherwise shows an ignorance and a lack of continued education on their part.

My primary is Dr. Joseph Kozlowski and I cannot review him or his practice with only 5 stars. Were it possible, I would rate with 10. Though smaller than many other offices I've visited for other reasons, his clinic is always clean and kept organized. He, his nurses and staff are professional, courteous, while friendly. His interest in my wife and me has been nothing short of admirable as has his training of future physicians.
