The Orthopedic Medical Center - Reseda provides our patients with office forms for your convenience. The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. A copy of the Power of Attorney and/or Advance Directive MUST be provided if someone else is signing the FORMS for the patient.
Dr. Jonathan Berkowitz is a fellowship-trained orthopedic sports medicine surgeon. He specializes in sports-related injuries. He provides total care of the shoulder, hip and knee, from minimally invasive arthroscopy to total joint replacement. Using the latest technology in both surgical and nonsurgical treatments, Dr. Berkowitz is dedicated to helping his patients get back to living a healthy, active lifestyle. He obtained his medical doctorate at the Carl Icahn Mount Sinai School of Medicine in New York City.
Dr. David Feingold is a board-certified orthopedic surgeon specializing in arthroscopic sports medicine, biologic joint preservation and joint reconstruction at his practice in West Hills, California. In addition to his private practice, Dr. Feingold also holds several hospital appointments, including at West Hills Hospital and Medical Center, Northridge Hospital Medical Center, Providence Tarzana Medical Center and the Specialty Surgery Center of Encino.Dr. Feingold received his medical degree from New York Medical College, and completed his internship and residency training in Orthopaedic Surgery at Harbor-UCLA Medical Center.

The staff is good but the Dr Friedland was a joke! My husband had an injury and he barely checked or touched the injured area. On top of that next time he went in to have MRI results he walks in and asks my husband Remind me why you are here? Are you kidding me! How about he opens his patient's file and actually does his job, act like you are interested in your patients and read what is going on. Horrible! We left and will never go back! If you want a subpar Dr. go here.

Medication they gave my elderly mother almost killed her. She ended up in the hospital for many days. I would give them 0 stars if it was possible.

I became quite a regular at this office as a patient of Dr. Perry this past year. Anyhow, after years of struggling with a problematic ankle, I finally had surgery on it done by him and the results have been good so far. Everyone at the office is very kind and dedicated, literally. They are good about checking people for Covid symptoms including taking temperatures. Dr. Perry is great and his diagnostic skills reflect the many years he's been fixing foot and ankle issues.

He didn't even need to send me for an MRI and knew what the problem was immediately (MRIs are time-consuming and often unnecessary I've found and insurance often will give substantial pushback before agreeing to pay for them). I went to a few other doctors over the years prior to my finding Dr. Perry and none were real interested in fixing the problem, instead recommending more conservative treatments. I'm glad to have this behind me now!

The one thing I would suggest they do is close the doors of the exam rooms. There are a few other doctors who work there, and they work with mostly older people who come in with a litany of complaints that they often loudly bark out. Also, some of them have personal problems that they tend to whine about to the receptionist. Other than that, the office is great.

Nice waiting area with automatic doors and usually you can find a place to park near the front door unlike other medical offices where you have to park, walk to the building, take an elevator, walk down the hall, etc. Not real convenient if you're on crutches haha Thanks again guys, I appreciate it!

My experience at Orthopedic Medical Center was excellent. The staff was amazing and pleasant which is very rare these days. Dr. Friedland and his assistant were helpful and accommodating answered all my questions. I highly recommend this place to everyone that is looking for a good Orthopedic. My wait time was short after filling out paperwork and straight to the point In conclusion, front desk and back office get a 10 out of 10 in my opinion.

8 I have seen three other Orthopedic Specialist and gave all three of them excellent reviews. This place was so terrible I can't even begin to tell you. The people out front where men and the doctor was mean, the doctor did not want to be there at all and could care less about my x-rays and the lady that was checking me out just wanted me out of there and wanted me to pay and leave as soon as possible. I am so disappointed and would probably not come back here if my life depended on it.

Great experience as a first timer. I barely waited to be helped when I checked in. The staff and Dr. Feingold were all very friendly and helpful. I especially loved how Dr. Feingold really took his time to explain my conditions with me as well as my treatment options. I had a great visit and the doctor's demeanor has really put me at ease so I highly recommend this clinic.

To all the staff at Orthopedic Medical Center, I thank you for the opportunity to just say thank you for all the years of unbelievable care, kindness and support. I have been seen over the years by several of the Dr's and always had a great experience. My go to Dr. however is Dr. Friedland. He has been there for me through the easy visits as well as the more difficult ones and can honestly say he is a special human being.

Always caring, listening to my issues and helping me get through them. Overall my experiences have been welcomed with a smile, warm reception and kind words. Thank you for all of that.

I went there for my elbow dislocation, the doctor didn't even look at my elbow he just look at the x-ray! And said "come back after 2 weeks!" That was it! I paid $153 to hear that!? And that's it! Are you kidding me! Don't go there anymore.

I have been a patient of Dr. Friedland's since 1978. He has done a couple of procedures on my knee, and in 2000 did a complete replacement. My wife and 4 kids are patients and now even some of our grand kids and spouses. He is the best. Very knowledgeable, thorough, and professional. I have recommended several friends to him who also swear by him.

His partner, Dr. Feingold operated on my cousin's shoulder recently and he was terrifc as well. When my son had a serious injury in Mammoth CA, on a weekend he consulted by phone with the surgeon in Bishop CA who did the very complicated surgery on his hand.

My wife, son and I have had several injuries (foot, back, wrist, arm, shoulder and elbow) in the past few years and we have all been very happy with the treatment by Dr David Feingold. Earlier this year I had surgery done on my elbow by Dr Feingold and my elbow is now perfect - the recovery was exactly as he said it would be. 6 months later and I am back at gym and playing golf. I highly recommend Dr Feingold for all your orthopedic needs. He is also very pleasant and caring which I unfortunately cannot say about all doctors that I have been to.

Wish i never got recommended to this place. He just want the consultation to go as quickly as possible, didn't even bother to look at the detailed historical records i brought with me because i want to see difference between injury to now. Pushed me to get a knee brace even when i told him i already had one, saying insurance would cover it without checking. then i see the bill for $110 in the mail. Only reason for 2 stars is the staff nurse was nice and helpful.

Great doctor, Dr. Friedland. Very good medical treatment. I am a long time patient, but with my Work Comp injury to both knees, service is very slow and sometimes requires multiple phone calls for follow-up to get treated. It can be quite frustrating!

The good: Dr. Friedland seems very knowledgeable. And is, generally, a great doctor. The bad: You will be rushed through your appointment. And don't bother calling the billing department, they don't seem to answer the phone and they don't return messages. The ugly: Apparently it's becoming a new thing for doctors in the state of California to require their patients to sign an arbitration form in order to ever even be seen (that's correct, folks, you are asked to sign away your rights before ever even laying eyes on the doctor!) Now this might be fine for simple procedures, however, when it comes to more complex ones like surgery I have a big problem with this requirement. IMO, that's what malpractice insurance is for, to protect the doctor in the event that he/she makes an egregious error.

I really am not ok with that burden being put onto the patient in the form of binding arbitration. All people seeking healthcare in the state of CA should be on high alert with signing these forms and refuse to do so whenever possible.

The staff are terrific. Dr. Friedland actually took the time to listen when I asked him to. They're very thorough in testing, and conservative in treatment. But why only three stars? Because Dr. Perry, the podiatrist, was the most disrespectful doctor I've ever met. He interrupted me constantly, complained about the X-ray techs, demanded to know who'd diagnosed me with arthritis. (Dr. Friedland.) He is knowledgeable, but combative and rude.

When I expressed my desire to be physically active again, he was dismissive. He never suggested exercises I could do, or referred me for physical therapy, but suggested I restrict calories. I ended up in tears. The office manager and a nurse were very kind about listening, and offered to have Dr. Perry talk to me again, but why? He'd clearly already made up his mind about what my problem was, given me terrible advice, and made me feel stupid for expressing a desire to be able to be active and fit. (Something that would make most doctors very happy!) I see that some people love Dr. Perry, even the office manager thinks he's great.

If you're comfortable with a smart doctor who's impatient and rude, he's the guy. Stick to Dr. Friedland. He's rushed, but he'll answer questions and listen.

At first I thought this place was great. Then I wised up after my poor son was NOT getting better after 6 months. I feel really bad for not realizing this sooner. I should have taken my son somewhere else after he was not better after two months of going to this place. But I thought I could trust these doctors. They claim to be experienced and experts.

I spent a lot of time and money here. They recommended custom orthotics for my son. They took a mold of my sons feet, then they made expensive custom orthotics for his flat feet / all of which I paid for. They also put leg braces on my 13 year old son for runners knee. He never got better. And I of course paid for all of this trusting they know what they are doing.

My son suffered with these leg braces most of the school year - in fact it was from November till March. They never once recommended physical therapy. Finally, I realized I was being too trusting of these people and I took my son to another orthopedic surgeon. They immediately recommended physical therapy. They also said he should NOT be in leg braces because that just makes his legs weaker and worse.

And they said he could just wear over-the-counter orthotics that are sold in retail stores such as Fleet Feet, Sports Authority, etc. My son was walking without leg braces that same day with orthotics we bought from them. And after 2 weeks of physical therapy he was walking comfortably. And after 6 weeks he was running! Do not take your young athletic children here.

This place seems to make their money treating old people and they use old methods that do not work. Also, the complaints about the people at the front desk or who answer the phones is correct - they are incompetent.

One star because Friedland was straightforward. Only qualm about him is that he did not provide me a chance to give informed consent regarding a AIS shot. Shitty office. Im usually called in 30-40 mins past my appt time. (He usually sees me only for 5-8 mins) I had to point out twice during one time how long i have been waiting. They did not apologize and had to voice that they were disrespecting my time and the essence of APPOINTMENT TIMES.

DO NOT COME HERE! It took me THREE weeks to book and appointment for my Mom. Once you call they transfer you to a voice message for you to leave your info promising to call back which they don't. Finally I came in here with my Mom since she was referred by her Doctor. The second I stepped foot I here a male employee (chubby, with glasses, hispanic, about 6ft. tall give or take) is literally SHOUTING at a patients family member.

Even though the patient's family member is upset and trying to ask questions I felt sympathy for her because of the bad moment her family member is going through. This patient's family member was not even shouting, she was expressing how she felt about the staff in this facility not helping her, and for this a-hole to come up to her and say "calm down or i'm going to call the police" is not the right way to reply to ANYONE in a full lobby facility unless they are attacking you.

The reason my Mother came in is because her back hurt. Well guess what? they injected her knees and did nothing about her back. The pain is still there. The staff is NOT professional. They all seem like headless chickens not knowing WTF they are doing. This place should SERIOUSLY be audited by some sort of government entity.

Wish i would've read more review or done more research but unfortunantly i had a complete major ACL ligament tear and minor meniscus so kinda felt rushed and pressured. dr. feingold didnt make this emotion any better. he rushed into my waiting room rushed thru the whole explaining and i didnt even feel the chance or the time to ask any questions. even though my vibe was completely off i still decided to have the entire procedure done. didnt like how he made me feel almost compared to an inmate worse like lost of freedom. the surgery went well had issues and never fully got an answer now i have knee problems again and im in need to find someone who will be the best for the job.
