Primary Care Coaltion The Primary Care Coalition envisions a strong, vibrant community that supports all people in achieving healthy lives. Our mission is to improve the health of vulnerable individuals and families by building partnerships and strengthening systems. We work with community partners and advocate for access to affordable, high quality health care and wrap-around services for people who face economic, linguistic, and other barriers to better health. The PCC works to improve community health by building new partnerships and strengthening existing ones.
Services
The Primary Care Coalition is a 5013 nonprofit organization. Donations to PCC are tax deductible to the fullest extent allowed by law. A copy of our current financial statement is available by contacting the Primary Care Coalition.
The Primary Care Coalition (PCC) makes health happen. We work with clinics, hospitals, health care providers, and other community partners to coordinate health care and related services for people experiencing vulnerability. The story of the PCC began in 1993, when a group of physicians and public health officials imagined a community where every resident had access to high-quality health services.
Our executive team works with diverse stakeholder groups to develop systems of care that meet the needs of community members who are experiencing vulnerability. We leverage the capabilities of policy makers, hospital systems, primary care providers and social service organizations, and coordinate planned and thought-out integration of health care systems with an eye to improvement. The Primary Care Coalition (PCC) is always looking to improve health care through building new partnerships and strengthening existing ones.
