All of the doctors and medical staff at Salisbury Pediatric Associates are highly trained professionals. Our physicians are board certified by the American Board of Pediatrics. Our providers are trained to address the special needs of infants, children, adolescents, and young adults. We provide comprehensive well-child care and individualized treatment options for a variety of healthcare needs.

Many of our providers have received advanced traning in integrative and functional medicine from prestigious institutions like the University of Arizona's Integrative Medicine fellowship.

Here you can find forms that are required by our office, as well as other forms used by the school system, for your convenience. There is information regarding important office policies, etc. There is also a tab to choose that will lead you to our Patient Portal. We are very excited to offer our patients this easy and convenient way to have access to their children's health history 24 hours per day! We will update this section regularly, so check back often.
We have now grouped the required forms depending on the age of establishing patients. Look below and find your appropriate group and print ALL forms that are listed except for the HIPPA- Notice of Privacy Practices. That is there for you to print if you would like but is not required for the initial visit to our office.
New patients must call our office to schedule your initial new patient appointment. After you have become an established patient at our office, you will be able to set up your personal Patient Portal-Online Account. From there, you can request appointments, request medication refills, and ask our staff and providers any questions, etc.
Your Health Plan usually requires you to pay a co-payment/co-insurance and/or meet a yearly deductible. These payments are expected at the time of service. The adult that accompanies your child to the office must bring the payment with them. In most cases, you will need to reschedule your child's appointment if you are unable to pay at the time of service.

Very dissatisfied with this place. Our child has a viral infection and instead of trying to figure out what it is exactly they start ordering things that are not going to help. He's currently at another doctor who in 5 minutes has tested him for 20 different virsus and we will have an answer tomorrow. This clinc should be shut down

Very dissatisfied & disappointed with with practice & some of their policies. I have 3 child. My oldest 2 fully vaxed and my youngest son whom I wished to not vax until he was old enough I could determine his level of functioning & gain a solid baseline. The practice gave no notice & discharged not only him, but my other two fully vaxed children. Not to mention all the other countless issues I've had with them that I let slide over the years, but this one takes the cake. Switched to a clinic that supports my CHOICES as a parent & not just trying to line their pockets with bonuses for every child in the practice being fully vaxed. Trust me doctors do get a compensation/kickback for this & is one of the only reason they push it so hard.

6 minutes late and the doctor refused to see her for a regular checkup they no longer do 15min grace period and haven't posted on website or notified clients that they inky do 5min. Disservice to the community. Also, months back I brought my child in for stomach pain they gave her meds and didn't help brought her back again still told me to wait. I took her to Atrium and they properly did testing and a pee test which they didn't do her and slove her issue.

Dr. Rose is unlike any other doctor I've met! The best way I can explain it is that she "just gets it". She is thorough and knows the right questions to ask to figure out what's going on. We drove over an hour to see her for integrative medicine and it was completely worth the drive. I recommend her to any child and parent who needs a doctor to find answers. The staff is also awesome and very responsive!

I heard wonderful things about this pediatrician office and switched my child to this practice. I've called nurse triage & have not received a call back for 3 days. I left a voicemail for the supervisor and have not received a call back either. I've tried to use the online portal but receive a message that my email is on file with the practice but the clinic has not activated the portal account.

I've left 2 messages over the course of 2 weeks with the IT department to address the online portal issue and still have not received a call back. I spoke to the receptionist who explained that everyone is very busy and just continue to call and leave voicemails for the triage nurse and someone will eventually call back. I work in healthcare and this is unacceptable and very disappointing.

The quality of care outweighed usually the review I am about to give- however the policies currently are absurd. I have two kids, in daycare during cold season- they refuse to vaccinate because he has a runny nose. during cold season. However instead of letting us know ahead of time, they ask you to do the covid questions, and wait until you're already there waiting to inform you.

They don't call you prior to the appointment (when they clearly have enough ladies doing NOTHING in the front office standing around *on the well side*) I completely understand the concern over Covid-19, however when you don't offer rapid testing to rule out the symptoms to be able to proceed with an appointment- that's poor practice. You have the tools to make your jobs easier and your patients time not wasted, and you don't utilize them OR your scheduling staff doesn't provide the options and you need to speak with them about that. Regardless- poor practice. Dr. Town's has run late, on the end of 45 minutes without any checking in or correspondence, letting us know and was disrespectful when I suggested that my time was not as important as his.

His bedside manner is also less than spectacular. My first son has a broken arm (humorous bone) at birth due to shoulder dystocia, and he decided to act like he knew what he was doing, and because he had never seen it, he wanted to take his wrap off and see. Michelle (NP) is usually pretty good, we liked her. Craighead is peculiar but we liked him too. Cody is good- nice, professional. Koontz is my favorite and clearly has a passion for the job he does. He will never make you feel less than or like you don't do enough. He praises the parents and the kids. Such a wonderful doctor and we really will miss him.

After years of my daughter being tossed around from doctor to doctor and none of them seemed to know what the previous doctor did or even prescribed, then not seeing her because of their 5 minute late policy (FACT), I had to find a new doctor ASAP. She has asthma and needed medicine but "Dr. Rose" said that 5 minutes was too long to wait. I guess she forgot that we waited 45 minutes in the room to see her the last visit.

I went there as a child as did my wife, sister and other daughter and it has gotten progressively worse. There are 3 great doctors there but but they are booked up over 3-4 months out.

I have been going to Salisbury Pediatrics since my 17 month old daughters first check up. I am a first time mother and had so may questions and often called the nurse during the first six months. I would often get nurse Brenda and she has been a tremendous help to me. She is always kind, caring and comforting when answering my questions. She would often tell me to call as many times as I wanted or call back after a couple days to check on my daughter.

She takes care of patients and reassures parents that they are doing a great job. I hope she is being recognized for the job she does because she surely deserves it! Thank you, Erin and Mia Crow
