Services
From minor scrapes to heart care, we'll help you get the care you need, when and where you need it. Our services include primary care and three specialty services. We also provide extended and after-hours care for non-emergency situations. A primary care physician (PCP) will coordinate all of your health care, including preventive care, physical exams and referrals to specialists.
Our patient portal is a secure online tool that lets you see your personal health records and communicate with your doctor. You can review doctor's visits, medications, test results and more. It's secure, easy to use and available 24 hours a day. Using the patient portal is entirely up to you. To sign up, contact your doctor's office. If you prefer not to sign up, it won't affect the care you receive from any provider in our group.
Your primary care physician (PCP) will oversee all of your health care, including preventive care, physical exams and referrals to specialists. Family practice and internal medicine physicians are available as PCPs. To find a San Bernardino Medical Group PCP in your insurance network, contact your health plan provider.
If you have a PPO plan, make sure the doctor you see is in your network. To find out, call your insurer and give them our tax ID number (95-3088615). If that doctor isn't in your network, you will be billed for out-of-network charges in addition to your copayment. If you are an uninsured or a self-pay patient, you have the right to request a good faith estimate of expected charges prior to receiving services. You or your representative may ask for a copy of the guidelines we use to approve or deny referrals.

RUN AWAY FROM THIS PLACE. WORST CARE FROM SUPPOSEDLY DOCTORS. DONT HAVE ANY TYPE.OF EMPATHY TO THEIR PATIENTS AT LEAST NOT WITH MY MOTHER. NOT HER DR. LEE JACKSON OR URGENT CARE. DR. TUPAS.URGENT CARE WAITED 3 HOURS TO.BE SEEING FOR HIM TO GIVE US 5 MINUTES OF HIS TIME AND SAID I REFERRED HER TO ORTHOPAEDICS SHE HAS TO WAIT FOR INSURANCE TO REAPOND FOR A APP. MEANWHILE MY MOM HAS BEEN IN PAIN FOR OVWR A WEEK. TEAR MENICUS. HER DR. FAITH LEE JACKSON NO APP AVAILABLE TILL FEBRUARY. LEFT HER MESSAGES EXPLAINING THE SITUATION ASKING HER FOR MEDICATION FOR PAIN. DISNT3BOTHWR ON CALLING MY MOM OR GIVING HER MEDICATION. No EMPATHY no Respect. Looking for another clinic that hopefully will give my mom the care she deserves. INSIRANCW IS NOT CHEAP AND THIS CLINIC IS HORRIBLE.

I have been trying to call the office for almost 8 months now, first to get the link for a appointment I had, secondly to cancel all appointments because I could never get anyone on the phone and 3rd to pay this co-pay I keep getting a bill for. How am I suppose to even pay this if the automated service recording take me in circles and no one calls me back? I tried to signup for the patient portal to pay my bill but that couldn't find me as a patient. Please call me to collect payment.

My monthly insurance premiums are over 2k but primecare is more interested in saving Blue Shield money than providing healthcare. Stay away from them!

Unfortunately, their level of service has declined to the point that they're almost useless. Patients are constantly left on hold for extended periods, and the system often just hangs up on us. Today, trying to set an appointment for a TB test, I was disconnected three times and then waited on hold for 24 minutes before someone finally answered. Then I was told I'd been dropped from the system because I hadn't been seen in two years

I'm new to this medical group. On SEVERAL occasions they DO NOT FOLLOW UP WHEN ITS CRUCIAL! Never call back when they say they will! As soon as possible I will change to another medical group. I DO NOT RECOMMEND. I changed from Kaiser at the beginning of this year and I regret it!

I was unhappy with my current doctor. I found these guys and thought I would give them a shot. I was extremely pleased all around. I didn't have to wait long at all on the phone. They set me up with an initial intake for a new pcp right by my home quickly. I also had an immediate need and was able to go to the Extended care program. I was in and out in less than two hours on a Friday (from initial sign in to walking out the door including X-ray and prescription pick up).

Everyone was friendly, understanding, and more than efficient. In the current state of doctor's offices of "hurry up and wait", I was extremely happy and would recommend these guys any day.

Horrible referral process and how they handle it. Told to wait 5 days for a referral for my newborn and if we don't receive it to give them a call. Sure enough I call every other day and I am given the same response day after day. Finally when my newborn has insurance they try to tell us that he needs to have his own insurance. He's a baby and is listed under our insurance!

As an update we are still waiting on the referral after nearly a mont. I can't wait to switch my sons doctor and out of this craphole. Go some place else with your kids parents.

Went to this drs office for 2 years after i was diagnised with diabetes. The dr was great although i had to wait 3 months for my initial appointment. 2 years later my dr retires and the staff cant get me in for my next checkup for 6 months because the drs that they have have too high of a patient load. Im now on my own finding a new dr at a different medical group because their "smooth transition to a new dr to maintain quality of care", as promised, was anything but smooth in my experance. Theyre staff dropped the ball. Im now paying the price for their incompantance. Theres no reason this should have happened. The office was aware that my dr was retiring months in advance and they should have made adaquate preparations concerning their patients.

Well I'd like to say that my experience with the X-Ray Department was very great. The individual who did my testing was very sweet and made me feel at ease and she assured me that everything would be okay. It Made me feel 100x better about myself and what I am experiencing. I really appreciate her assurance and definitely made me not feel as bad as I was feeling. Thank You your an angel :
