Insurance cards - A copy of the front and back for each child. Complete Immunization Records - Necessary for each child by the 1st Check-up. Medical Records - Any records from a previous physician, if applicable. Our practice was established over fifty years ago. It has expanded from a solo practice to one that is comprised of five Physicians and two Nurse Practitioners.

Expectant parents and parents of prospective patients are welcome to attend one of our monthly open house sessions. This allows you to see the office, meet several staff members, ask questions and see what Pediatric Care is all about.
