Provided below are various forms that require completion prior to your visit to our office. Please bring the completed form(s) with you to your scheduled visit. This will help expedite the registration process as a new patient to our office.

We are the official team physicians of the Grand Junction Rockies. Our physicians prepare the team for the season with thorough examinations including EKG, Cardiogram and physicals. Throughout the season we treat and help prevent player injuries. Western Orthopedics physicians are the official team physicians of Colorado Mesa University student athletes. Our Primary Care Sports Medicine department provides comprehensive screenings, education and treatment for athletes of all ages.

Please do not use this area to contact us regarding your personal medical issues, as it is not secure and could be read by others. Please call us with questions about your medications or medical treatment. After Office Hours: Our phone is answered 24 hours a day; a doctor is always on call. In case of an emergency, go to the emergency room of the hospital of your choice and have the ER staff contact the physician on call. If you are already an existing patient in our practice, your physician's group can be contacted via our Follow My Health Patient Portal.
