Our enrollment team is available via telephone, Monday - Friday to help patients enroll in Medi-Cal, Covered California, CalFresh, Medicare, and HealthPAC. Please call 925-462-1755 to make an appointment. In order to complete the enrollment process without contact, patients can transmit documents to Axis via a secure, encrypted link that Axis will provide you with.

Each Axis clinic is being maintained with extra cleaning and screening to ensure staff and patient safety. Axis Community Health provides comprehensive medical services, partnering with each of our patients to develop a personalized health care plan. We provide care for every member of the family, from newborns to seniors.

As Chief Medical Officer, Dr. Moody makes sure our patients receive quality care - including her own. She knew she wanted to work with children during her first visit to a pediatric clinic in medical school - "it just clicked, " she says. Whether you need a physical, urgent care or care for a chronic condition, our medical team will help you get and stay healthy. Screening and management of chronic medical conditions such as diabetes, high cholesterol, high blood pressure, asthma and heart disease. Our staff is available to help with enrollment in Medi-Cal or other programs than can cover services.

Health Connect by Axis, is an enrollment-based chronic care management service offered by Axis Community Health which is specifically for patients with chronic conditions. This service connects you with a Care Team Member who will help you manage your chronic conditions at home. Provider and care teams will ensure communication between all team members and specialists for which referrals are indicated.
