One form is your authorization for us to use the information you share with us, and the other is a PHI (Protected Health Information) compliance form which is required anytime we discuss your healthcare experience. Open the form using Adobe Acrobat Reader (or you will be able to convert to Adobe Acrobat Reader when you open the file).

CareLink is an Epic application designed for use by non-affiliated community providers to access their patients' records. This group includes referring physicians and members of their clinical, billing and coding staff; non-affiliated sub-acute and post-acute facilities, including their case managers; and health plans.
