Clinix medical providers are available six days a week to focus on both preventive and restorative care, working towards a goal of optimal health for all ages.

Our behavioral health specialist provides mentoring to Clinix patients who need support during life's challenges as well as connecting families with community resources.

For after hours urgent care when you are unsure of the severity, call Clinix Center for Health. We always have an on-call provider available to assist in resolving your health concerns. Please call if you feel there is a medical problem that cannot wait until the office reopens. Clinix on-call provider may resolve your issue over the phone, set up an office appointment or direct you to an emergency room, Urgent Care or After Hours facility. For immediate care, dial 911, or go to the nearest emergency room.

In 2017, Clinix was accepted into a 5- year pilot program called Comprehensive Primary Care Plus (CPC+). Medicare created this pilot to assist medical offices like ours to redesign our practices to better address the special needs of different patient populations. One of the hallmarks of this pilot is that it assists the practice in providing care management for high risk, high-need patients. This could include the elderly, those patients recently hospitalized or patients newly diagnosed with a chronic disease (such as diabetes).

NCQA is a private, non-profit organization dedicated to improving health care quality.
