The consultation appointment is the essential first step when considering plastic surgery. We will send you an email with new patient forms that we request you complete and return to us prior to your appointment so we can create your new patient account. On the day of your appointment, please bring one form of identification, such as a driver's license, and your insurance cards if you are considering a procedure that might be covered by insurance.We require a $50 payment in advance to schedule a consultation for every procedure that is self-pay, non-insurance.
