Please complete our paperwork at your convenience prior to your appointment. If you rather bring the completed paperwork with you to your appointment, please arrive 15 minutes early. You will also need to bring your insurance card(s) and photo ID. It is important that you bring your MRI, CT, and X-Ray images with you on an imaging cd to your appointment.
