Please call in advance to schedule check-ups. We require all of our patients to follow CDC recommendations to receive all of the vaccines required by Georgia schools, unless we determine there is a genuine medical contraindication. Below is the current check up and vaccine schedule. The links bring up important Vaccine Information Statements (VIS) with important information for you to review before your visit.
After Hours
Report
Our physicians are always available for emergencies after hours. If you need assistance for an urgent medical issue after hours, please call the regular office numbers. For non-urgent matters, please send us a non-urgent message or schedule an appointment via the portal. Your call will be taken by our answering service and then triaged to either a nurse advisor or to one of our physicians. If the emergency or problem can not be resolved by the nurse, the physician on call will be notified to call you.

Child Registration for children 18 and older: Demographic form for new patients 18 or older. Non-Parental Caretaker Registration: This is for children who will be brought to the office by someone other than a parent or legal guardian, giving us permission to see the child with another adult. Insurance
Report
We will gladly file insurance for patients who participate in our managed care plans after we have verified their benefits. In order to file for you, we require proof of insurance prior to the appointment with the physician. Please contact the office to determine if we are providers for your managed care plan.
