As a courtesy, Monarch provides benefit checks to estimate what your carrier covers for our programs.
Homekit
Report
We prescribe vitamin injection kits for self-injection. Homekits are shipped to your home after a review of your medical history by our Practitioner. Every kit is a 30-day supply. A Telemedicine consultation with a Practitioner is required and included before the shipping of each order. Homekits are currently only available for Oregon residents.
