Appointments for adult medicine, pediatrics, behavioral health and substance use counseling, health insurance enrollment, and advice with how to effectively use your medications are available by phone.Our pharmacy is able to deliver your medications directly to your doorstep; no need to wait in line at a community pharmacy! This health center received HHS funding and has Federal Public Health Service (PHS) deemed states with respect to certain health or Health-related claims, including medical malpractice claims for itself and its covered individuals.

ChapCare has been putting patients first since 1998. For 24 years, ChapCare has served the communities of the San Gabriel Valley, providing quality healthcare to men, women and children of all ages. Since 2013, ChapCare has been recognized as a Patient Centered Medical Home (PCMH). A PCMH is a team-based model of care led by a physician who provides continuous and coordinated care throughout a patient's lifetime to maximize health outcomes.

Patients are assigned to a physician in ChapCare's network and work closely with a team to ensure comprehensive care, regardless of ability to pay.
