Huntington Plaza Pediatric Group is excited to announce that our practice is now offering a patient portal and a mobile app for our families! Using the portal allows secure, convenient communication between you and our office. This type of communication allows us to deliver secure documents at your request, and also allows you to access your child's medical chart from any portable internet device or the privacy of your home. Important Note: The patient portal is meant for communicating NON-URGENT medical needs and concerns only.

If you are giving birth at Huntington Memorial Hospital, please inform hospital admissions that you are choosing Huntington Plaza Pediatrics as your primary physicians. Be careful while describing the name of the practice as there is another practice with a very similar name. You can choose any physician from our group and all of them see patients interchangeably.

Developmental Questionnaire - This questionnaire will help the doctors understand your child's development. If your child is 0-8 years of age, please download this questionnaire and bring it in when you come for your child's well visit/Annual physical. Developmental Screening for Kids 0-8 yrs. Depression Questionnaire - This questionnaire is for children age 11-21 years of age and will help the doctors understand your t(w)eenager's stress and mental development.Post-Natal Questionnaire - This questionnaire is meant for the mother/caregiver and will help the doctor's understand the factors that will help/hinder the baby's development.
