Please note: If you are attempting to pay a bill for Broadalbin Perth Family Health, the Surgical Health Center or Charlton Urgent Care, you will need to follow the instructions on your bill due to online bill pay not being an option for those sites of care.

You may mail or fax forms to request records or send a hand-written letter signed and dated stating the records you need and where to send the records with a number to reach you. You may also call the office to request your records or results by secure e-mail, sign-up for the patient portal, arrange for pick-up or to be mailed to your home.
