Your team will include a designated Primary Care Provider that will be responsible for your continuous and comprehensive care. This provider will lead the team of other medical professionals who collectively are responsible for your ongoing care. Your medical team will help coordinate your care ensuring that you receive personalized care using the latest technology and evidence-based medical care.

To best serve your healthcare needs and to make your visit as pleasant as possible, we have compiled the following information. In an effort to reduce the amount of time you spend in our reception area, all patient visits are by appointment. We do keep appointments for same day scheduling to accommodate urgent and emergency needs of our patients.

These forms are available for download in.pdf format. Forsyth County Schools and surrounding counties require children trying out for school sports to have a physical exam annually. Camps and day cares may also require physical exams every one or two years. Even though we may see your child for other reasons throughout the year, these visits are not considered complete physicals and cannot be used to complete your child's form. If your child desires to participate in school sports or attend camp, and if a physical is needed, please call early to schedule an appointment.
