Dr. Spencer is a Doctor of Osteopathic Medicine; in addition to full medical training, he has studied musclo-skeletal (joints, neck, back) medicine. In addition, he is board certified in Family Medicine after training at the University of Minnesota. I enjoy the broad range of family medicine and the opportunity to get to know my patients and their families.

Patients under 18 must be accompanied by a parent or guardian. If a parent or guardian cannot be present, this consent form must be signed before the patient can be seen.

All copays are due at the time of service. If you are unable to meet your payment obligations we can reschedule you to a more suitable time. We accept multiple insurance plans; patients must verify coverage directly with their plan and the patient accepts ultimate responsibility for payment. The patient is responsible for providing necessary information for insurance claims and for working with our office to make payment arrangements when necessary.All charges are ultimately the patient's responsibility, regardless of insurance coverage.
