Many people in our community need personalized health care services, in-home care, and affordable housing. We provide homes and health care services to meet the needs of people in our community such as individuals with developmental disabilities and older adults, enabling them to maximize their independence and be part of our community. We provide a full continuum of health care services: disability services, senior housing, skilled nursing, therapy, home health, and hospice care. In addition, we offer a wide range of market rate & income-restricted rental housing communities.

Rosa Camille Williams dedicated her life to caring for those in need. She was a nurse who provided care in hospitals and in homes during the early 1900s. She was the inspiration for her daughter, Margaret, and granddaughter, Mary T, to provide services to improve the lives of others in their community.
Minnesota Disability Services

We provide dignified care and rehabilitation for physical or developmental disabilities in the environment that is right for you. We offer our services in one of our homes, in your own home, or in the community. Our goal is to foster community connections and social interaction in a warm, supportive environment.
Individualized Service Options

We offer Individualized Service Options throughout the Twin Cities Metro and St. Cloud areas. Our goal is to support an individual's independence in their own living environment. We provide unit based services including 24 hour emergency assistance which provides both security and the flexibility to enable support to be customized on an individual basis. Each person has a support plan that is developed collaboratively in a person-centered approach.
Residential Services

We provide a comfortable and dignified residential home environment to provide services to individuals with physical or developmental disabilities. The people we serve are integrated into the community and live life to the fullest. Our residential homes are located in the community to allow us to provide services in a community-based setting, instead of an institutional environment.
