This online portal allows you to view and download records, update your demographic information and medical history, and send secure messages to the office. If you do not have a patient portal account, please call the office at 360-779-2336 to get started.

Insurance Information

When scheduling an appointment, please have your insurance information readily available. If you are scheduling a routine eye exam (including preventive vision checks or to update a glasses/contact lens prescription), the appointment will be billed as routine vision. If you are scheduling for a medical reason (including diabetic eye exams, glaucoma screening, and urgent/acute eye issues), the appointment will be billed as medical eyecare.

We are located just west of the new Safeway Shopping Center, across highway 305 on 8th Avenue. Turn to the west at the Lincoln Rd. / Iverson St. traffic light. Next right onto 8th Avenue and we are the third building on the right.
