To sign up for FollowMyHealth, or to log into your Optum Care account, click "Sign in" below. Once you've setup your account, it does take up to 24 hours for your medical records to appear in FollowMyHealth. No medical records have been lost in the process of changing patient portals. No, any e-vists that were in My Health Online will not show up in the new FollowMyHealth patient portal.

Primary care

Now, some of your primary care visits can happen wherever you are. Virtual visits through NowClinic enables you to have a face-to-face video appointment with your provider, via phone, tablet, or computer. At Southwest Medical, you have Team Medicine on your side at one of our healthcare centers. It's important not only to find a primary care physician and team that's close by, but one that you'll be comfortable with.

Specialty care

First, you and your primary care provider (PCP) will decide that you need to see a specialist and give you a referral. Then you can choose a provider from within our group of specialists. Your PCP will work with your specialty care providers to make the best decisions for your overall health. For instance, if you have a constant, long-term health problem, called a chronic condition, you'll need more specialty care.

Urgent care

At Southwest Medical, we've taken the Urgent Care concept several steps further. We understand that medical visits can't always wait. That's why our Urgent Care centers are open seven days a week and most holidays (hours may change). We also have one Urgent Care center with a 23-hour observation unit. If we can treat your problem within 23 hours, you can avoid going to the emergency room for further treatment.
